that is, they offer customer service, adjudicate claims, and make

Establish an account on their website and you will be able to see
your claims and order additional insurance cards.

Caremark Is our Pharmacy Benefits Manager (BPM).
The PBM manages the Trust’s prescription drug plans. If you go to
www.caremark.com and set up an account, you can see which
local pharmacies participate in the Caremark family of pharmacies.
You will also be able to learn more about the drugs you take, obtain
personalized prescription forms, refill prescriptions online, and
compare costs.

Dental administers the Dental PPO. To receive the best
benefits, you should see dentists who subscribe to the network.
